Five retailers are voluntarily recalling crib tents and play yard tents made by Tots in Mind, Inc. due to concerns about their safety, the U.S. Consumer Product Safety Commission said Wednesday.

The CPSC has received reports of 27 tent failures – one resulting in death and another resulting in a serious brain injury. The tents present an entrapment and strangulation hazard to small children, the commission said.

Tots in Mind, Inc. is no longer in business and has stopped selling the tents.

Amazon.com, Bed Bath & Beyond/Buy Buy Baby, Burlington Coat Factory, Toys R Us/Babies R Us and Walmart are pulling the tents from their shelves and offering refunds or store credit to customers who bought the tents.
